Case : Barrett esophagus inspection

Organ : Esophagus

Patient Information : 85-year old man, referred with visible lesion in Barrett esophagus

Medical History : Upper gastrointestinal endoscopy for long-standing complaints of heartburn revealed a visible lesion in Barrett esophagus

1. High-resolution white light endoscopy, proximal part of the lesion

This image high-resulotion white light endoscopy image shows the Barrett mucosa and suspicious, slightly elevated lesion extending from 3-6 o’clock.

2. Narrow-band imaging and near focus, proximal part of the lesion

Detailed inspection of the elevated lesion using NBI and near focus shows an irregular mucosal and vascular pattern suspicous for cancer.

3. High-resolution white light endoscopy, distal part of the lesion

White light endoscopy shows that the lesion extends distally from 2- 4 o’clock, where elevated and slightly irregular mucosa is seen.

4. Narrow-band imaging and near focus, distal part of the lesion

Using NBI and near focus this part of the lesion also shows an irregular mucosal and vascular pattern suspicious for neoplasia.

Case video

This video shows a pull back in white light, where the Barrett segment is seen with a suspicous lesion from 2-4 o’clock distally, extending to a more elevated lesion from 4-6 o’clock more proximally. Upon more detailed inspection using NBI and near focus, the mucosal and vascular pattern can be assessed, which confirm the suspicion of neoplasia.
